parapligia-is weakness or paralysis of both Lower limbs due to pyramidal tract legion UMNL or LMNL flassid para pligia0 انا عندي نوعين اما intra medullary او extra medullary اعرفهم ازاي وافرق بينهم علي اي اساس ده اللي هقوله اولا لازم نعرف فقرات الظهر بتتمثل ازاي في latral spino thalamic tract الترتيب من الخارج للداخل اولا العجزيه ثم القطنيه ثم الصدريه ثم العنقيه وبناء علي القاعده دي لو الحاله extra medullary اذن يضغط الاول علي العجزيه اذن لو اختبرت احساس المريض ف منطقة الsacral مش هيحس وهتكون دي اول علامه والعكس ف intra هيحس لكن بعد فتره من تقدم المرض هتتاثر المنطقه دي 0000 ثانيا ال bladder fiber بتمشي في inner part of medulla اذن لو الحاله intra هتضربها اولا وهتصبح اول شكوي المريض انه مش بيتحكم في تبوله والعكس في extra هتكون شكوي متاخره .0000 ثالثا ال motor weakness of intra medullary is symmetrical يعني لو عملت اختبار قوة اي عضله هتدي نفس الدرجه in two limbs والعكس في الextra medullary رابعا ال sensory manifistation usually jacket loss at intra medullary with dissociated sensory loss )loss of pain and temp with preservationof touch but extra medullary sensory level below which all types of sensation are lost
